A worker was fatally crushed at a worksite in Sydney, resulting in a company being fined over half a million dollars. Mishaal Prasad, a boilermaker, was involved in moving a 9000kg steel beam with an overhead crane when it fell on him at a Smithfield site in July 2020. The company, MHE-Demag Australia, was sentenced on Tuesday after pleading guilty to failing to comply with health and safety regulations. They were involved in the designing, manufacturing, and maintenance of mechanical handling equipment. Prasad was tasked with fabricating steel structures and was grinding the sharp edges of the beams when the incident occurred. The beams became unstable during the process, causing them to fall on Prasad. Despite efforts to help him, he suffered fatal injuries. SafeWork NSW conducted an investigation and charged MHE-Demag Australia. Although the company had risk management systems in place, they lacked a separate documented system of work for the task Prasad was performing. The company was aware of the risks involved in the task and failed to take sufficient steps to prevent the incident. As a result, they were fined $525,000. MHE-Demag Australia has since ceased trading and transferred its assets and employees to Konecranes Pty Ltd.
