Authorities have confirmed one man has died, and three people were critically injured.

“A patient at the scene was unfortunately deceased and couldn’t be revived by NSW Ambulance paramedics,” ambulance spokesperson Melissa Hill said.

The man is yet to be formally identified.

One of the injured is a woman aged in her 30s who was carefully cut free from the wreckage of her car.

She suffered head injuries and was airlifted to St George Hospital in a critical condition.

Two men are also fighting for life with multiple injuries at Westmead Hospital and Liverpool Hospital.

The three men had been travelling in the truck.

Nine ambulances and three rescue helicopters were sent to the scene in what Hill said was a “large response”.

The injured patients were unable to provide any insight into the incident at the scene.

Fire and Rescue NSW Superintendent Adam Dewberry earlier described the situation as an “extremely serious crash”.

Dramatic pictures taken from the 7NEWS chopper showed contents from the truck scattered across the road and over the safety barrier.

The site will be examined by specialist police from the Crash Investigation Unit on Friday afternoon.

“The road is expected to remain closed for some time and the public is urged to avoid the area,” police said.

Bulli Pass is also closed southbound at the M1 Princes Motorway.

A report will be prepared for the coroner and anyone with information should contact police or Crime Stoppers.
